3/27/2023 EnchantaVaughn Approved Behavior: QMHP-C arrived to the home of Client. Upon arrival to the home setting, Client appeared to be frustrated and irritated when QMHP-C arrived at the residence. Mom said she needed Client to stop acting like it’s the end of the world. Client has failed to decrease behaviors associated with ADHD. Mom reported Client has had anger issues but recently noticed her anger has developed into insecure and low confident behaviors. Client exhibits poor listening skills, talking over the adult instructing her at the time. Behavior: Client presented positive behaviors throughout the remainder of the session needing minimal redirection from QMHP-C to focus as she was withdrawn from the session at times. 1.Intervention: QMHP-C conducted session today on decreasing disrespect for authority figures and taking ownership for her own actions. QMHP-C probed Client about her behaviors yesterday. QMHP-C probed Client about her ability to not get involved with a negative crowd. QMHP-C discussed with Client the importance of conducting herself appropriately at all times. QMHP-C utilize positive self-talk strategies to help Client think positively at all times and to talk about her actions and how they will make people feel. QMHP-C utilized active listening skills while Client was verbalizing information about her day. QMHP-C encouraged Client to remain positive and more positive things will happen. Intervention: QMHP-C prompted Client to refrain from impulsive, hyperactive, defiant, and aggressive behaviors especially running, refusing authority instruction, and interrupting others who are trying to learn. QMHP-C utilized behavior modification strategies including removing Client from the stimulant environment in efforts for her to reduce inappropriate behaviors. QMHP-C will monitor effects of meditation and coping practices daily and record progress and improvements. QMHP-C explored the Client’s capability to cope with her stressors and to get back on track. QMHP-C assisted Client with finding and utilizing necessary resources at home. Response: Client was not receptive to counselor’s prompts and redirections and after a period of time resulted to behaving very erratically and becoming physically violent as well. Client has a history of displaying aggressive, non-compliant behaviors towards authoritative figures in the home and/or community setting. This has been the first occasion in which Client has displayed this level of behaviors in the presence of counselor. Client was throwing objects, yelling loudly, at mother and counselor. Client was highly oppositional during session and displayed argumentative and frustrating behavior. Response: Client was acceptive of extra help and support. Client utilized the resources provided and seem to be doing better with managing symptoms of ADHD. Client returned to activity ready to work and was more motivated to try and stay positive for the remainder of the day. Client displays moderate participation unless she’s distracted. Client recognized when she’s not paying attention and quickly corrected herself. Mom enjoyed seeing Client do the right thing for once. After several strategies and restraint by counselor.
